Travel Risk Assessment

If you are travelling abroad, please make sure you contact us at least 8 weeks before your date of travel, to allow time to arrange any vaccinations that may be necessary. You must complete the assessment form before booking an appointment with the nurse for travel vaccines, as we have to order some of them in. We do not offer non-NHS travel vaccines or malaria prophylaxis. For these, or if you are travelling within the next 8 weeks, you will need to go to a travel clinic elsewhere.
